High School & Junior High Winter and Spring 2022-2023 Sports Physicals:

Wednesday November 2th, 2022

6:00 pm - 8:00 pm

All attendees MUST have (or they will be denied entrance):

1) an appointment - click SignUpGenius 

2) exact change or check for $20 - make check payable to ETSD and write athlete's name, grade, and sport on memo line 

3) successfully registered for their winter sport on PlanetHS.com 

4) Preprinted Section 6 Health History form, completed, and blank Section 7 exam form - you can only print this from the system once you have successfully registered at PlanetHS.com.  Open Athletic Forms, click on Section 7, click “print/upload”, and choose “print”. DO NOT print Example Forms.  No printing will be available on-site.

Masks are now optional.

All athletes are responsible for uploading a picture of their Section 7 to their PlanetHS portal after leaving physicals.  All other "forms" should be completed online vs uploading pictures of them.  Paper copies will not be accepted. Please upload forms sooner rather than later!

Please remember that all athletes must register on PlanetHS.com, and link accounts with their parent/guardian prior to sports starting (independent of this appointment process for physicals).  If you experience trouble using this system, or you or your child have multiple accounts (which will cause the system to disable all of your accounts), please contact schoolsupport@planeths.com - Exeter Athletics staff are unable to assist in this process.
