Spring Sports Physicals

Wednesday February 24th

All attendees MUST have (or they will be denied entrance):

1) an appointment - click SignUpGenius

2) exact change or check for $20 - make check payable to ETSD and write athlete's name, grade, and sport on memo line

3) successfully registered for their winter sport on PlanetHS.com

4) Preprinted Section 5 Health History form, completed and blank Section 6 - you can print this from the system once you've successfully registered at PlanetHS.com. No printing can be done on-site.

All attendees are required to wear a mask that covers their nose and mouth at all times.

All athletes are responsible for uploading their Section 6 to their PlanetHS portal after leaving physicals. All other "forms" should be completed online. Paper copies will not be accepted.

Please remember that all athletes must register on PlanetHS.com, and link accounts with their parent/guardian prior to sports starting (independent of this appointment process for physicals). If you experience trouble using this system, please contact their support service by using the button on their page - Exeter Athletics staff are unable to assist in this process.
